TBAE CEPH Log

All registrants are required to record and maintain their CEPH on a CEPH Log. You may download the CEPH Log (PDF) or call or write to the TBAE office to request a copy of the log. Please do not submit your CEPH log with your renewal payment. You will be asked to provide this log if you are chosen for an audit. The logs should be maintained in your records for a period of not less than five years.
